UPDATE ON CHANGES TO ODSP FORMS (OCTOBER, 2021)

Recently, the Ontario government changed all the forms in the Disability Determination Package for Ontario Disability Support Program (ODSP) applicants. But while the forms have changed, the legal test that decides whether a person is eligible for ODSP has not changed.
To qualify as a person with a disability, most applicants must fill out the Disability Determination Package (DDP) that ODSP sends to them.
The new DDP forms filled out by health professionals have revised rating scales. Health professionals use the scales to rate an applicant's intellectual and emotional wellness, and to rate the impact the disability has on the applicant's daily activities.

Canadian Council on Rehabilitation and Work (CCRW)
CCRW helps persons with disabilities find a job or career, helps employers hire persons with disabilities and supports employers with disabilities. As well, the CCRW provides Job Accommodation Services by working with employers across Canada to create healthy and inclusive workplaces.
The CCRW also funds the Workplace Essential Skills Program which is designed specifically to provide post-secondary graduates and experienced professionals with disabilities with a professional view of the world of work.
